1. Download the Student Application Packet or vist building B behind the Tulalip Boys & Girls Club and pick one up during any scheduled After-School Academy Session.
2. Fill out and sign the Application, Activity Authorization, Media Release and Behavioral Contract with your student(s).
3. Submit the packet to any of the Mentors at TEAM-WORKS Academy · 7707 36th Avenue NW, Portable B Tulalip, Washinton 98271 · FAX: (206) 323-1118 OR submit the forms to the front desk of the Education Building.
NOTE: In case of an emergency, we will need to contact the student's parents/guardians. Please be sure all contact information included in the form is up-to-date and contact us with any changes that occur throughout the school year.
